To order by mail or fax, simply download and print out our New Customer form. Fill the forms out and mail them to the address below. Or fax them to our toll-free fax number.

Once all required forms are received, a Customer Service Representative will contact you to confirm your order.

For prescription items please fax in your prescription(s) along with the New Customer Forms. For Over the Counter item(s) we do not require a prescription.

All faxed orders will be confirmed and processed within 48 hours of receiving your fax. If you don't have a fax machine, ask your doctor's office if they will fax the forms for you along with the prescription(s), or use a fax service available at many postal or office services.
