Fetzima at CanadaPharmacyOnline.com

The brand Fetzima (containing the active ingredient levomilnacipran) is manufactured by AbbVie Corporation in Canada.

Generic levomilnacipran is currently unavailable (at this pharmacy).

Dosage

Fetzima is commonly available in 20 mg, 40 mg, 80 mg, and 120 mg oral capsules at this pharmacy.

Your doctor will determine the appropriate dose based on your individual needs and medical conditions.

Usage

Fetzima (levomilnacipran) is a prescription medication used to treat major depressive disorder (MDD) in adults. It is a serotonin and norepinephrine reuptake inhibitor (SNRI) that works by affecting the balance of certain natural substances (serotonin and norepinephrine) in the brain that are thought to be unbalanced in people with depression. Fetzima is intended for regular, daily use for the management of chronic depression and is not used for the management of fibromyalgia.

Mechanism of Action
Fetzima (levomilnacipran) is a selective serotonin and norepinephrine reuptake inhibitor (SNRI). While the exact mechanism by which Fetzima exerts its antidepressant effect is unknown, it is thought to affect the levels of serotonin and norepinephrine in the brain by blocking their reuptake by nerves in the central nervous system. This action enhances serotonergic and noradrenergic activity, which is believed to contribute to its antidepressant effects.

Take Fetzima exactly as your doctor prescribes, following the instructions in the product monograph in Canada. Consult your doctor for personalized guidance and safe use.

The information below summarizes some, but not all, potential side effects, risks, and interactions. Before taking Fetzima, it is essential to discuss all possible side effects, risks, and interactions with your doctor or pharmacist.

Side Effects

The information below is a partial list of side effects; individual reactions can vary. Before starting Fetzima, it's crucial to discuss potential side effects and risks with your doctor or pharmacist.

Common side effects - The following are some common side effects of Fetzima; other side effects may occur. Contact your doctor promptly if any side effects are persistent, worsening, or concerning, or if you experience any other concerning symptoms.
• Abdominal pain
• Constipation
• Decreased appetite
• Dizziness
• Dry mouth
• Fatigue
• Hot flush/flushing
• Increased heart rate
• Increased sweating
• Insomnia (trouble sleeping)
• Nausea
• Orthostatic hypotension (dizziness or lightheadedness when standing up)
• Palpitations
• Sexual dysfunction (e.g., erectile dysfunction, ejaculation problems, decreased libido, delayed or absent orgasm)
• Vomiting

Serious Side Effects - These are rare but potentially severe side effects. Seek immediate medical attention or call emergency services if you experience any of the following:
• Abnormal bleeding or bruising: Including unusual nosebleeds, blood in urine, bleeding gums, or cuts that don't stop bleeding.
• Activation of mania or hypomania: Symptoms may include decreased need for sleep, an elevated or irritable mood, racing thoughts, and increased energy.
• Angle-closure glaucoma: Symptoms include sudden blurred vision, eye pain, redness of the eye, or seeing halos around lights.
• Discontinuation Syndrome: Symptoms that may occur upon abrupt discontinuation or dose reduction, including dizziness, headache, nausea, anxiety, agitation, tremors, sensory disturbances (e.g., "electric shock" sensations), and insomnia.
• Hyponatremia (low sodium levels): Symptoms include headache, confusion, weakness, muscle twitching, or seizures.
• Sustained increases in blood pressure or heart rate: Your doctor should monitor your blood pressure and heart rate regularly. Pre-existing hypertension must be controlled before starting Fetzima.
• Seizures
• Serotonin syndrome: A potentially life-threatening condition. Symptoms can include agitation, hallucinations, confusion, fast heartbeat, dizziness, flushing, tremors, stiff muscles, muscle twitching, seizures, nausea, vomiting, or diarrhea.
• Suicidal thoughts or behaviours: Especially in young adults (24 years and younger) and at the beginning of treatment or when the dose is changed. This is a boxed warning for antidepressants.
• Urinary hesitation or retention: Difficulty urinating or inability to empty the bladder.
• Severe Allergic Reactions (Hypersensitivity): Rash, hives, swelling of the face, lips, tongue, or throat, difficulty breathing. Long-term use of Fetzima requires ongoing monitoring due to potential effects on blood pressure and heart rate. Other long-term concerns may include persistent sexual dysfunction and, as with other similar medications, a potential increased risk of bone fractures.

Contraindications

The following is not an exhaustive list of contraindications. Before starting Fetzima, it is crucial to discuss all your medical conditions, past and present, and all medications you are taking with your doctor or pharmacist.

Fetzima should not be used if you:
• Are hypersensitive to levomilnacipran, milnacipran, or any of its ingredients.
• Are currently taking a Monoamine Oxidase Inhibitor (MAOI) or have taken one within the last 14 days. This includes MAOIs for psychiatric disorders (e.g., phenelzine, selegiline), and other MAOIs like linezolid (an antibiotic) or methylene blue (a dye). Taking Fetzima with an MAOI can lead to severe, potentially fatal, serotonin syndrome. You should also wait at least 7 days after stopping Fetzima before starting an MAOI.
• Have uncontrolled narrow-angle glaucoma.
• Have experienced a myocardial infarction or cardiac intervention within the past 12 months.
• Have NYHA Class III or IV congestive heart failure.
• Have uncontrolled tachyarrhythmia.
• Have uncontrolled hypertension.
• Have a history of cerebrovascular accident.

Tell your doctor about all of your medical conditions, past and present, before starting Fetzima. This is crucial because certain conditions affect whether Fetzima is right for you. These conditions may include, but are not limited to:
• Bipolar disorder, hypomania, or mania
• Bleeding problems
• Diabetes mellitus (monitor blood glucose levels closely)
• Have a history of stroke or other cerebrovascular events.
• History of seizures or epilepsy
• Kidney problems: Dosage adjustment is necessary for moderate or severe renal impairment; Fetzima is not recommended for end-stage renal disease.
• Liver disease
• Low blood pressure (hypotension) or high blood pressure (hypertension)
• Low blood sodium (hyponatremia)
• Urinary hesitation or retention

Dizziness and Drowsiness
Fetzima may cause dizziness or fatigue. Avoid driving, operating machinery, or engaging in activities that require mental alertness until you understand how it affects you. Be cautious when standing up quickly, especially if you are an older patient, to reduce the risk of dizzy or fainting spells.

Pregnancy and Breastfeeding

The safety of Fetzima during pregnancy has not been fully established. It should be used during pregnancy only if the potential benefits outweigh the possible risks to the fetus. Neonates exposed to SNRIs (including Fetzima) late in the third trimester may experience complications such as respiratory distress, feeding difficulties, or temperature instability. It is known that levomilnacipran passes into breast milk, and the effects on a nursing infant are unknown. A decision should be made to discontinue breastfeeding or discontinue the drug, taking into account the importance of the drug to the mother.

Use in Children

The safety and effectiveness of Fetzima in children and adolescents under 18 years of age have not been established. Fetzima is not approved for use in children under 18 years of age. Antidepressants, including Fetzima, may increase the risk of suicidal thoughts and behaviours in some people 24 years of age and younger, especially during the initial treatment period or when the dose is changed.

Use in Older Populations

Elderly patients taking Fetzima should be monitored closely by their healthcare provider. Older adults may be more susceptible to specific side effects, particularly those related to the cardiovascular impacts (such as increased heart rate and blood pressure) and hyponatremia. Dose adjustments may be necessary in elderly patients with impaired renal function.

Drug Interactions

Before taking Fetzima, it is essential to discuss all potential risks and interactions with your doctor or pharmacist. For complete and up-to-date information, refer to the Patient Medication Information | AbbVie.

Provide your doctor with a complete list of all your medications, including prescriptions, over-the-counter (OTC) drugs, vitamins, and herbal supplements. It is also essential to inform your doctor about any herbal teas, recreational drugs, or significant dietary changes you are making, as these could potentially interact with Fetzima. Fetzima may interact with other medicines, but is not limited to:
• Anticoagulants (blood thinners): (e.g., warfarin, apixaban, rivaroxaban).
• Antiplatelet drugs: (e.g., clopidogrel, aspirin).
• Cyclobenzaprine
• Desmopressin
• Monoamine Oxidase Inhibitors (MAOIs): (e.g., phenelzine, selegiline, linezolid, methylene blue)
• Nonsteroidal Anti-inflammatory Drugs (NSAIDs): (e.g., ibuprofen, naproxen, indomethacin, aspirin).
• Opioid pain relievers: (e.g., fentanyl, tramadol, oxycodone).
• Other Antidepressants: (e.g., SSRIs like sertraline, paroxetine; other SNRIs like venlafaxine, duloxetine; tricyclic antidepressants like amitriptyline, nortriptyline).
• Potent CYP3A4 inhibitors: (e.g., certain antifungals like ketoconazole, itraconazole; certain HIV drugs like ritonavir, cobicistat). The maximum recommended dosage of Fetzima should not exceed 80 mg once daily when used with strong CYP3A4 inhibitors.
• Stimulants: (e.g., amphetamine/dextroamphetamine, methylphenidate).
• Triptans: (e.g., sumatriptan, zolmitriptan) used for migraine.
• Tryptophan supplements

It is generally recommended to avoid alcohol consumption while taking Fetzima. Alcohol may enhance some central nervous system effects of Fetzima, and consuming alcohol with extended-release medications can sometimes affect the release rate of the drug.
