Anoro Ellipta at CanadaPharmacyonline.com

Brand Anoro Ellipta (umeclidinium bromide/vilanterol trifenatate) is manufactured by GSK and is available in several countries worldwide, including Canada, Turkey, and the United Kingdom.

A generic version of Anoro Ellipta is currently unavailable (at this pharmacy).

Dosage:

Anoro Ellipta is available in one strength: 62.5 mcg of umeclidinium and 25 mcg of vilanterol per actuation (the delivered dose). In some instances, such as in the USA, it is also called 55 mcg/22 mcg. Each inhaler contains 30 doses.

Your doctor will determine the appropriate dose based on your needs and medical conditions.

Usage:

Anoro Ellipta is a long-term, once-daily maintenance treatment for chronic obstructive pulmonary disease (COPD), which includes chronic bronchitis and emphysema. It helps to ease breathing difficulties, such as shortness of breath, caused by COPD. It is crucial to understand that Anoro Ellipta is not a rescue medication and should not be used to treat sudden, acute breathing problems. A separate rescue inhaler (typically containing a short-acting beta-agonist like salbutamol or albuterol) should be used to relieve acute symptoms immediately.

Mechanism of Action:
Anoro Ellipta combines two active components: umeclidinium bromide and vilanterol trifenatate, each with a distinct mechanism of action.

Umeclidinium bromide is a long-acting muscarinic antagonist (LAMA), also known as an anticholinergic. Umeclidinium works by blocking the action of acetylcholine, a neurotransmitter that causes the muscles around the airways to tighten. It achieves this by binding to muscarinic receptors, primarily the M3 subtype, on the smooth muscle cells of the airways, leading to bronchodilation.

Vilanterol trifenatate is a long-acting beta2-adrenergic agonist (LABA). It works by stimulating beta2-adrenergic receptors on the smooth muscle of the airways. This activation leads to the activation of adenylyl cyclase, increasing the production of cyclic AMP (cyclic adenosine monophosphate). Elevated cAMP levels cause the smooth muscles of the airways to relax, resulting in bronchodilation, which opens the airways and improves airflow.

Combining these two long-acting bronchodilators with different mechanisms of action (anticholinergic and beta2-adrenergic), Anoro Ellipta provides a greater and more sustained improvement in lung function than using either medication alone. This synergistic effect helps to relieve symptoms, improve daily activities, and reduce the frequency of COPD exacerbations.

Use Anoro Ellipta exactly as your doctor prescribes, following the instructions in the product labeling information. Consult your doctor for personalized guidance and safe use.

The information below summarizes some, but not all, potential side effects, risks, and interactions. Before taking Anoro Ellipta, it is essential to discuss all possible side effects, risks, and interactions with your doctor or pharmacist.

Side Effects:

The information below is not an exhaustive list of side effects; individual reactions can vary. Before starting Anoro Ellipta, it is crucial to discuss potential side effects and risks with your doctor or pharmacist.

Common side effects - These are some common side effects of Anoro Ellipta, but others are possible. Contact your doctor promptly if any side effects are persistent, worsening, or concerning, or if you experience any other concerning symptoms.
• Common cold symptoms (e.g., runny nose, cough)
• Constipation
• Diarrhea
• Lower respiratory tract infection (e.g., bronchitis, pneumonia)
• Muscle spasms
• Neck pain
• Pain in the arms or legs
• Sinus infection
• Sore throat

Serious Side Effects:
Anoro Ellipta can also cause some less common but serious side effects. Seek immediate medical attention if you experience any of the following:
• Changes in laboratory blood values may occur, including potential increases in blood sugar (hyperglycemia) and decreases in potassium levels (hypokalemia).
• Chest pain - (may be a sign of cardiac problems or excessive stimulation from the LABA component).
• Heart problems, including increased blood pressure, fast or irregular heartbeat (palpitations), and feeling your heartbeat.
• High blood sugar levels (hyperglycemia) – symptoms may include increased thirst, frequent urination, and increased hunger.
• Low potassium levels (hypokalemia) – symptoms may include muscle weakness, muscle spasms, and irregular heartbeat.
• Nervous system effects, such as tremor and nervousness.
• New or worsening eye problems, including glaucoma (symptoms may include eye pain or discomfort, nausea or vomiting, blurred vision, seeing halos around lights, and red eyes).
• Paradoxical bronchospasm (unexpected narrowing of the airways, causing coughing and difficulty breathing immediately after using the inhaler). • Serious allergic reactions, such as rash, hives, swelling of the face, mouth, and tongue, and breathing difficulties.
• Sudden breathing problems: if you experience sudden wheezing, coughing, or shortness of breath immediately after using the inhaler, discontinue and seek immediate medical attention.
• Symptoms of excessive LABA use may include chest pain, increased blood pressure, fast or irregular heartbeat, headache, tremor, and nervousness. • Urinary retention, which can manifest as difficulty urinating, painful urination, frequent urination, or a weak urine stream.
• Worsening COPD symptoms: if your breathing problems worsen, do not increase your dose and consult your doctor.

Contraindications:

The following is not an exhaustive list of contraindications. It is crucial to discuss all your medical conditions, past and present, and all medications you are taking with your doctor or pharmacist before starting Anoro Ellipta.

Anoro Ellipta should not be used if:
• You are allergic to umeclidinium bromide, vilanterol trifenatate, or any of the other ingredients in Anoro Ellipta;
• You have asthma. Anoro Ellipta is specifically indicated for the treatment of Chronic Obstructive Pulmonary Disease (COPD) and should not be used to treat asthma. Long-acting beta2-agonists (LABAs), like vilanterol in Anoro Ellipta, can increase the risk of severe asthma exacerbations and asthma-related death when used alone (without an inhaled corticosteroid to treat airway inflammation). Anoro Ellipta does not contain an inhaled corticosteroid.
• You have a severe allergy to milk proteins (Anoro Ellipta contains lactose monohydrate, which may contain trace amounts of milk proteins).

Tell your doctor about your medical conditions, past and present, before starting Anoro Ellipta. This is crucial because certain conditions can affect whether Anoro Ellipta is right for you. These conditions may include, but are not limited to:
• Cardiovascular disorders, such as coronary insufficiency, cardiac arrhythmias, and hypertension. Vilanterol, a LABA, can have cardiovascular effects and may increase heart rate and blood pressure in some individuals.
• Conditions that predispose to hypokalemia (low potassium levels);
• Diabetes mellitus;
• Narrow-angle glaucoma - Anoro Ellipta can increase intraocular pressure;
• Seizure disorders;
• Thyrotoxicosis;
• Urinary retention – (primarily in men with prostatic hyperplasia or bladder-neck obstruction, as anticholinergics can worsen this condition)

Caffeine
It is advisable to limit excessive caffeine intake while using Anoro Ellipta. Your doctor may recommend reducing or avoiding large amounts of caffeine, as it could potentially exacerbate side effects such as tremors and an increased heart rate due to additive stimulant effects.

Drug Interactions:

Before taking Anoro Ellipta, it is essential to discuss all potential risks and interactions with your doctor or pharmacist. For complete and up-to-date information, refer to the official Anoro Ellipta prescribing information:
• Beta-adrenergic receptor blocking agents (beta-blockers): Non-selective beta-blockers (e.g., propranolol) can counteract the bronchodilating effects of vilanterol and may even induce bronchospasm in patients with COPD. While selective beta-1 blockers (e.g., atenolol, metoprolol) might have a less significant impact, caution is still recommended, and alternative beta-blockers may be considered if necessary.
• Certain antidepressants: Monoamine oxidase inhibitors (MAOIs) and tricyclic antidepressants (TCAs), such as amitriptyline, may increase the risk of cardiovascular side effects when used with Anoro Ellipta. Caution is advised, and alternative antidepressants might be considered. Nefazodone, a serotonin antagonist and reuptake inhibitor (SARI), has a potentially different interaction profile, so consultation with your doctor is recommended. Selegiline, primarily an MAO-B inhibitor, may have a lower interaction risk at typical doses, but caution is still warranted.
• Decongestants and stimulants: These medications may increase the risk of cardiovascular side effects when used with Anoro Ellipta due to their sympathomimetic properties.
• Drugs that prolong the QTc interval: Certain antiarrhythmics (e.g., amiodarone), some antipsychotics (e.g., aripiprazole, quetiapine), and macrolide antibiotics (e.g., erythromycin) are known to prolong the QTc interval. Vilanterol can also have this effect, so concurrent use of these medications may increase the risk of severe heart rhythm disorders, such as torsades de pointes.
• Non-potassium-sparing diuretics: Loop diuretics (e.g., furosemide) and thiazide diuretics (e.g., hydrochlorothiazide) can lower potassium levels. Beta-agonists, such as vilanterol, can worsen this effect, potentially increasing the risk of cardiac arrhythmias. Careful monitoring of potassium levels is advised.
• Other long-acting beta-agonists (LABAs): Using Anoro Ellipta with other LABAs (e.g., salmeterol, formoterol, arformoterol, olodaterol, indacaterol) may increase the risk of LABA-related side effects, such as chest pain, increased blood pressure, and a fast or irregular heartbeat. Concurrent use should be avoided.
• Other anticholinergics: Using Anoro Ellipta with other anticholinergic medications, including short-acting (e.g., ipratropium) and long-acting (e.g., tiotropium, aclidinium) agents, may increase the risk of anticholinergic side effects, such as dry mouth, constipation, difficulty urinating, and blurred vision. Caution is advised.
• Strong CYP3A4 inhibitors: Drugs such as ketoconazole, itraconazole, voriconazole, and specific HIV protease inhibitors (e.g., ritonavir, cobicistat) can significantly increase the levels of vilanterol in your body by inhibiting its metabolism. This may elevate the risk of systemic side effects associated with LABAs. Caution is recommended, and alternative treatments might be considered during concurrent use.

Pregnancy and Breastfeeding

Anoro Ellipta should only be used during pregnancy if the potential benefit to the mother outweighs the potential risk to the fetus. If you are pregnant, think you may be pregnant, or are planning to have a baby, do not use Anoro Ellipta without asking your doctor. Inform your doctor immediately if you become pregnant while taking this medication. It is not known whether umeclidinium or vilanterol trifenatate passes into human breast milk. Therefore, it is essential to consult your doctor before breastfeeding while using Anoro Ellipta to assess the risks and benefits.
