Olmetec Information

Olmetec and Olmesartan Medoxomil at CanadaPharmacyOnline.com

The manufacturer of brand-name Olmetec (olmesartan medoxomil) available at this pharmacy is Organon Canada Inc. (Kirkland, Québec). Daiichi Sankyo originally developed Olmetec and has been marketed in Canada under various entities over time.

Generic olmesartan medoxomil is available in various markets, including Canada and internationally. It is produced by several reputable manufacturers that meet high regulatory standards for safety and quality. The specific manufacturer of the medication dispensed may vary based on pharmacy inventory. Generic olmesartan medoxomil is sourced from manufacturers that meet applicable regulatory standards.

Important Information about Generic Olmesartan:

Generic olmesartan medoxomil tablets contain the same active ingredient as brand-name Olmetec. They must meet the same effectiveness and quality standards set by Health Canada. Studies have shown that generic olmesartan is bioequivalent to brand-name Olmetec, meaning it has similar absorption and achieves comparable plasma concentrations. However, individual responses to medication can vary due to factors such as metabolism, genetics, and other health conditions.

Dosage:

Olmetec (olmesartan medoxomil) tablets are available in the following strengths and forms:
• Tablet: 20 mg and 40 mg oral tablets

The dose of Olmetec is individualized based on the patient’s blood pressure response. The usual recommended starting dose is 20 mg once daily. For patients who require further blood pressure reduction after 2 weeks of therapy, the dose may be increased to 40 mg once daily.

Usage:

Olmetec (olmesartan medoxomil) is indicated for the treatment of mild to moderate essential hypertension. Olmetec may be used as monotherapy (on its own) or in combination with other antihypertensive agents. It may be combined with a thiazide diuretic (such as hydrochlorothiazide) for additional blood pressure lowering when monotherapy is insufficient. The fixed-dose combination of olmesartan medoxomil and hydrochlorothiazide is available separately as Olmetec Plus. Olmetec may be used in a wide range of hypertensive patients, including those with diabetes, chronic kidney disease (with appropriate dose adjustment), left ventricular hypertrophy, and patients who have experienced a stroke. Olmesartan does not cure hypertension. Treatment is typically long-term, and blood pressure may begin to fall within the first two weeks of therapy.

Mechanism of Action
Olmesartan medoxomil, the active ingredient in Olmetec, is an Angiotensin II Receptor Blocker (ARB). It lowers blood pressure by selectively targeting a hormonal pathway that regulates fluid balance and blood vessel constriction. Olmesartan is a selective AT1 receptor antagonist, meaning it binds directly to AT1 receptors and blocks angiotensin II from binding. Because angiotensin II cannot bind to these receptors, blood vessels relax and widen, and the kidneys stop retaining excess sodium and water. With relaxed vessels and less fluid volume, blood pressure drops significantly.

Take Olmetec exactly as your doctor prescribes, following the instructions on the product monograph. You can take Olmetec with or without food. Take the medicine once daily, at the same time each day.

The information below summarizes some, but not all, potential side effects, risks, and interactions. Before starting Olmetec, it is essential to discuss all possible side effects, risks, and interactions with your doctor or pharmacist.

Side Effects:

The information below outlines potential side effects of Olmetec (olmesartan medoxomil). It is not an exhaustive list, and individual reactions can vary. While many side effects of olmesartan occur regardless of dosage, certain adverse reactions—such as dizziness or light-headedness caused by low blood pressure—can be dose-dependent and may become more noticeable when increasing the dose from 20 mg to 40 mg.

Common side effects: These are common side effects of Olmetec; other side effects are possible. Contact your doctor promptly if any side effects are persistent, worsening, or concerning, or if you experience any other concerning symptoms.
• Back pain
• Diarrhea
• Difficulty sleeping (insomnia)
• Dizziness or light-headedness, particularly when standing up quickly (orthostatic hypotension)
• Fatigue or tiredness
• Headache
• Upper respiratory tract infection (common cold symptoms)

Serious Side Effects: Seek immediate medical attention or call emergency services if you experience any of the following symptoms:
• Anaphylaxis or severe allergic reaction: Hives, rash, swelling of the face, lips, tongue, or throat, or difficulty breathing. Angioedema (facial swelling) has been reported with olmesartan; anaphylactic reactions have been reported very rarely.
• Chest pain or ischaemic events
• Hyperkalaemia (high potassium): Muscle weakness, fatigue, palpitations, or an irregular heart rate.
• Hypotension (severe): Marked dizziness, near-fainting, or fainting.
• Renal failure: Decreased urine output, significant swelling, or signs of acute kidney injury.
• Severe skin reactions: Stevens-Johnson syndrome or toxic epidermal necrolysis (rare)
• Sprue-like enteropathy: Severe, chronic diarrhea (often watery) with substantial, unexplained weight loss, which may develop months to years after starting olmesartan.

Contraindications:

The following is not an exhaustive list of contraindications. Before starting Olmetec, it is crucial to discuss all your medical conditions, past and present, and all medications you are taking with your doctor or pharmacist.

Do not take Olmetec/olmesartan medoxomil if:
• Aliskiren (Rasilez) in patients with diabetes or moderate-to-severe renal impairment: The combination of Olmetec with aliskiren is contraindicated in patients with diabetes mellitus or moderate-to-severe renal impairment due to an increased risk of hypotension, syncope, stroke, hyperkalaemia, and deterioration of renal function.
• Allergies: Known hypersensitivity to olmesartan medoxomil or any ingredient in the formulation.
• Breastfeeding.
• Pregnancy.

Tell your doctor about all of your medical conditions, past and present, before starting Olmetec. This is crucial as certain conditions affect the drug’s safety.
• Bilateral renal artery stenosis or renal artery stenosis in a single functioning kidney
• Diabetes mellitus
• Heart failure
• Hepatic impairment (moderate to severe)
• Hyperkalaemia or conditions predisposing to high potassium (e.g., renal impairment, concurrent use of potassium-raising drugs)
• Hypertrophic cardiomyopathy with outflow tract obstruction
• Primary hyperaldosteronism
• Renal impairment
• Significant aortic or mitral valve stenosis
• Volume or salt depletion

Drug Interactions:

Before taking Olmetec, discuss all potential risks and interactions with your doctor or pharmacist. For complete, up-to-date information, please refer to the patient medication information at https://www.organon.com/canada-en/wp-content/uploads/sites/5/2021/05/OLMETEC-CI_E.pdf.

Provide your doctor with a complete list of all your medications, including prescriptions, over-the-counter (OTC) drugs, vitamins, herbal supplements, recreational drugs, and any significant dietary changes, as these may interact with Olmetec. Olmetec may interact with other medicines, including but not limited to:
ACE inhibitors (e.g., ramipril, enalapril, perindopril, lisinopril): Concurrent use of Olmetec with ACE inhibitors creates dual blockade of the renin-angiotensin-aldosterone system (RAAS). This combination is generally not recommended, as it does not provide additional cardiovascular benefit compared with either agent alone, but significantly increases the risk of hypotension, syncope, stroke, hyperkalaemia, and acute renal failure. In patients with diabetic nephropathy, the combination has been shown in clinical trials to be harmful.
Alcohol: Potentiates the hypotensive effects of olmesartan and increases the risk of dizziness and fainting.
Aliskiren (Rasilez): Contraindicated in patients with diabetes or moderate-to-severe renal impairment. In other patients, the combination is generally not recommended due to increased risks of hypotension, hyperkalaemia, and renal impairment.
Antihypertensive agents and vasodilators: Additive blood-pressure-lowering effects when Olmetec is combined with any other antihypertensive drug, including calcium channel blockers, beta-blockers, diuretics, alpha-blockers, or nitrates. Dose adjustments may be required.
Colesevelam (a bile acid sequestrant): Colesevelam has been shown to significantly reduce the area under the curve (AUC, which measures overall drug exposure) and peak blood levels of Olmesartan when administered together, reducing olmesartan’s efficacy. Olmetec should be administered at least 4 hours before colesevelam to avoid this interaction.
Hydrochlorothiazide and other diuretics: When olmesartan is combined with diuretics, the combination produces additional blood pressure-lowering effects.
Lithium: Reversible increases in serum lithium concentrations and toxicity have been reported during concomitant administration of lithium with ACE inhibitors and ARBs. Concurrent use is generally not recommended. If the combination is essential, monitor lithium levels carefully.
NSAIDs (non-steroidal anti-inflammatory drugs, e.g., ibuprofen, naproxen, indomethacin) and selective COX-2 inhibitors: In patients who are elderly, volume-depleted, or have impaired renal function, concurrent use of ARBs and NSAIDs may further deteriorate renal function and cause acute renal failure. Monitor blood pressure and renal function if NSAIDs are used concurrently.
Potassium-raising drugs (potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium, heparin, trimethoprim): Concurrent use with olmesartan increases the risk of hyperkalaemia. Monitor serum potassium regularly.

Pregnancy and Breastfeeding:

Olmetec is contraindicated during pregnancy. Use of ARBs during the second and third trimesters of pregnancy is known to cause serious fetal and neonatal injury, including hypotension, neonatal skull hypoplasia (underdevelopment of the skull), anuria, reversible and irreversible renal failure, and death. When pregnancy is detected, Olmetec must be discontinued as soon as possible, and the patient should contact their doctor immediately. Patients of childbearing potential who are taking Olmetec should use effective contraception. Olmetec is contraindicated during breastfeeding. It is not known whether olmesartan medoxomil is excreted in human milk. Because of the potential for serious adverse effects in the nursing infant, breastfeeding must not be initiated or should be discontinued while taking Olmetec.

Use in Children:

Olmetec is not approved for use in patients under 6 years of age. For children aged 6 to 16 years, dosing is weight-dependent: the recommended starting dose is 10 mg once daily for patients weighing less than 35 kg, and 20 mg once daily for patients weighing 35 kg or more.
