Alvesco at CanadaPharmacyOnline.com

Brand Alvesco is marketed and distributed by Covis Pharma in Canada.

Generic ciclesonide is currently unavailable (at this pharmacy).

Dosage

Alvesco is commonly available in 100 mcg and 200 mcg inhalation aerosols at this pharmacy.

Your doctor will determine the appropriate dose based on your individual needs and medical conditions.

Usage

Alvesco (ciclesonide) is a prescription medication used for the long-term control and prevention of asthma symptoms in adults and children 12 years of age and older. It is an inhaled corticosteroid that works by reducing inflammation in the airways, helping to keep them open and making breathing easier. Alvesco is not a rescue inhaler and should not be used to treat sudden asthma attacks or acute bronchospasm. It is intended for regular, daily use to manage chronic asthma.

Mechanism of Action
Alvesco (ciclesonide) is a corticosteroid that acts as a prodrug. Once inhaled, ciclesonide is converted by esterases in the lungs to its active metabolite, des-ciclesonide (C21-desisobutyryl-ciclesonide or M1). This active metabolite is a potent glucocorticoid that binds to glucocorticoid receptors in the lung, leading to local anti-inflammatory activity. By inhibiting the release of inflammatory mediators and reducing the infiltration of inflammatory cells in the airways, Alvesco helps to reduce airway hyperresponsiveness, mucus production, and edema, thereby alleviating asthma symptoms.

Take Alvesco exactly as your doctor prescribes, following the instructions in the patient information leaflet. Consult your doctor for personalized guidance and safe use.

The information below summarizes some, but not all, potential side effects, risks, and interactions. Before taking Alvesco, it is essential to discuss all possible side effects, risks, and interactions with your doctor or pharmacist.

Side Effects:

The information below is a partial list of side effects; individual reactions can vary. Before starting Alvesco, it's crucial to discuss potential side effects and risks with your doctor or pharmacist.

Common side effects - These are some common side effects of Alvesco, but others are possible. Contact your doctor promptly if any side effects are persistent, worsening, or concerning, or if you experience any other concerning symptoms.
• Arthralgia (joint pain)
• Back pain
• Cough
• Dry mouth and throat
• Gastroenteritis
• Headache
• Hoarseness
• Nasal congestion
• Nasopharyngitis (swelling of the nose and throat)
• Nausea, vomiting
• Oral thrush (a fungal infection in your mouth)
• Pain in arms, legs, and back
• Sinusitis (swelling of the sinuses)
• Sore throat
• Upper respiratory infection

Serious Side Effects - These are rare but potentially severe side effects. Seek immediate medical attention or call emergency services if you experience any of the following:
• Adrenal insufficiency or decreased adrenal gland function: Symptoms include tiredness, weakness, dizziness, or persistent nausea or vomiting.
• Decreased bone density (weakened bones)
• Decreased adrenal gland function
• Eye problems: Glaucoma, cataracts, or increased intraocular pressure.
• Paradoxical Bronchospasm: Sudden wheezing or difficulty breathing immediately after using the inhaler.
• Severe allergic reactions: Rash, hives, angioedema (swelling of the face, lips, tongue, or throat), difficulty breathing.
• Systemic eosinophilic conditions, including Churg-Strauss Syndrome, may occur, especially when reducing oral corticosteroid doses.
• Weakened immune system and increased risk of infection (e.g., chickenpox, measles, tuberculosis, herpes becoming active again).

Long-Term Side Effects - Long-term use of Alvesco may lead to:
• Chronic oral thrush
• Decreased bone density (weakened bones)
• Glaucoma and cataracts
• Slowed growth in children

Contraindications

The following is not an exhaustive list of contraindications. Before starting Alvesco, it is crucial to discuss all your medical conditions, past and present, and all medications you are taking with your doctor or pharmacist.

Alvesco should not be used if you:
• Are experiencing an acute asthma attack or status asthmaticus. Alvesco is not a rescue inhaler and will not provide immediate relief for sudden asthma symptoms.
• Have had a definite allergic reaction to ciclesonide or any of its ingredients.
• Have moderate to severe bronchiectasis.
• Have untreated fungal, bacterial, or tuberculosis infections of the respiratory tract.

Tell your doctor about all of your medical conditions, past and present, before starting Alvesco. This is crucial because certain conditions affect whether Alvesco is right for you. These conditions may include, but are not limited to:
• Adrenal insufficiency: Patients transitioning from oral steroids to Alvesco should be monitored closely, as severe and sometimes fatal withdrawal symptoms may occur.
• Certain infections: Alvesco may weaken the immune system, increasing the risk of getting new infections or reactivating latent ones. Use with caution, if at all, in patients with active or quiescent tuberculosis infection of the respiratory tract; untreated systemic fungal, bacterial, viral, or parasitic infections; or ocular herpes simplex. If exposed to contagious diseases like chickenpox or measles, contact your doctor immediately.
• Eye problems (e.g., glaucoma, cataracts, increased ocular pressure): Alvesco can worsen these conditions. Regular eye exams may be recommended.
• Hypothyroidism.
• Cirrhosis and/or severe hepatic impairment (liver disease).
• Recent nasal ulcers, nasal surgery, or nasal trauma until healing has occurred.
• Systemic corticosteroid effects: While less common than with oral steroids, high doses or prolonged use can lead to systemic effects such as hypercorticism (e.g., Cushingoid features) or growth suppression in children.
• Bone mineral density reduction: Long-term use can lead to a decrease in bone mineral density.
• Oropharyngeal Candidiasis: To reduce the risk of oral thrush, rinse your mouth thoroughly with water and spit it out after each inhalation.

Drug Interactions

Before taking Alvesco, it is essential to discuss all potential risks and interactions with your healthcare provider, such as your doctor or pharmacist. For complete and up-to-date information, refer to the product monographs from Canada: https://pdf.hres.ca/dpd_pm/00058252.PDF.

Provide your doctor with a complete list of all your medications, including prescriptions, over-the-counter (OTC) drugs, vitamins, and herbal supplements. It is also essential to inform your doctor about any herbal teas, recreational drugs, or significant dietary changes you are making, as these could potentially interact with Alvesco. Alvesco may interact with other medicines, but is not limited to:
• Certain HIV Drugs (e.g., ritonavir, nelfinavir): Similar to antifungals, these can increase Alvesco's levels and side effect risk.
• Clarithromycin: Can increase the levels of Alvesco's active metabolite.
• Specific potent CYP3A4 inhibitors, including some antifungals (e.g., ketoconazole, itraconazole): These can affect how Alvesco is broken down by the liver, potentially increasing levels of its active metabolite and the risk of side effects.

Alcohol consumption is not known to interact with Alvesco directly, but both can cause some similar side effects (e.g., headache, nausea, dizziness). Alcohol consumption may also increase the risk of dehydration.

Pregnancy and Breastfeeding

The safety of Alvesco during pregnancy has not been fully established. It should be used during pregnancy only if the potential benefits outweigh the potential risks to the fetus. It is also unknown if ciclesonide passes into breast milk, so caution should be exercised when used in breastfeeding women. Poorly controlled asthma during pregnancy can pose significant risks to both the mother and the unborn baby.

Use in Children

The safety and effectiveness of Alvesco in children under 12 years of age have not been established. Alvesco is indicated for use in adults and children 12 years of age and older. Long-term use in children and teens may affect growth, and regular growth checks may be needed.
