Trulance (plecanatide) is a prescription medication used in adults to treat certain types of constipation, including Irritable Bowel Syndrome with Constipation (IBS-C), a chronic condition characterized by abdominal pain and constipation, and chronic Idiopathic Constipation (CIC), Long-lasting constipation where the cause is unknown.

Mechanism of Action
Trulance (plecanatide) is a guanylate cyclase-C (GC-C) agonist. It is structurally similar to uroguanylin, a naturally occurring peptide in the gastrointestinal tract. Plecanatide binds to and activates GC-C receptors on the luminal surface of intestinal epithelial cells. This activation leads to an increase in both intracellular and extracellular concentrations of cyclic guanosine monophosphate (cGMP). The increase in cGMP stimulates the secretion of chloride and bicarbonate into the intestinal lumen. It decreases sodium reabsorption, leading to an influx of water into the intestines. This increase in intestinal fluid helps to soften stools, increase bowel movement frequency, and reduce abdominal pain and bloating associated with IBS-C and CIC. Trulance acts locally in the upper gastrointestinal tract with minimal systemic absorption.

• Abdominal pain
• Abdominal distension (bloating)
• Diarrhea (most common, usually mild)
• Dizziness
• Flatulence (gas)
• Nausea
