Usage

Orap (Pimozide) is a prescription medication that reduces uncontrolled movement or outbursts of words or sounds in people with Tourette's syndrome. Pimozide suppresses tics' physical (motor) and vocal (phonic) symptoms when these symptoms interfere with daily life function. The active ingredient in the Orap tablet is Pimozide. Pimozide belongs to the group of medicine called Antipsychotics (1st generation). Pimozide works by decreasing the activity of a natural substance known as dopamine (D2 receptor antagonist) in the brain.

Side Effects and Precautions

The common side effects of Orap are drowsiness, akinesia, dry mouth, sedation, constipation, impotence, speech disorder, and visual disturbance.

You may have withdrawal symptoms if you stop using Orap medication suddenly; however, you should contact your doctor right away if you have any of the serious side effects such as:
• A seizure (convulsions);
• Fast or pounding heartbeats, fluttering in your chest, shortness of breath, and sudden dizziness (like you might pass out);
• Fever, chills, tiredness, mouth sores, skin sores;
• Reduced sexual activity in males or inability to produce sperm and enlarged breast in females;
• Recurrent vomiting;
• Severe nervous system reactions such as very stiff muscles, high fever, sweating, confusion, fast or uneven heartbeats, tremors, feeling like you might pass out;
• Uncontrolled muscle movements in your face (chewing, lip-smacking, frowning, tongue movement, blinking, or eye movement)

Warnings and Interactions

This medication is available as a tablet and should be taken with water, usually once daily at bedtime. You should use this medication as directed by your doctor. Do not increase your dosage, as this may worsen the side effects. Call your doctor if you are sick with vomiting or diarrhea. You may develop an electrolyte imbalance, which could cause heart rhythm problems while taking Pimozide.

Use this medication regularly to get the most benefit from it. To help you remember, take it at the same time each day. Do not stop taking this medication without consulting your doctor. Your condition may become worse when the drug is suddenly stopped. Your dose may need to be gradually decreased. You should limit your alcoholic beverages while using Pimozide tablets. Drinking alcohol can increase certain side effects of Pimozide. It would be best if you did not take grapefruit as it can worsen the side effects of this medication.

You need to have a regular clinic visit for follow-up. Your doctor will need to check your progress regularly. Your heart function may need to be checked using an electrocardiograph. It may take several weeks before your symptoms improve. Keep using the medication as directed and tell your doctor if your symptoms do not improve or if they get worse.

Before you start using Orap, inform your doctor about your medical history, especially if you have:
• An enlarged prostate or urination problems;
• Glaucoma;
• Heart problems, high blood pressure, a heart attack, long QT syndrome;
• Kidney disease;
• Liver disease;
• Seizures or epilepsy

Tell your doctor about all your medications, including prescriptions, over-the-counter medications, herbal products, and vitamin supplements. Tell your doctor if you are pregnant or plan to become pregnant. Taking Pimozide during the last 3 months of pregnancy may cause problems in the newborn, such as withdrawal symptoms, breathing problems, feeding problems, fussiness, tremors, and limp or stiff muscles. However, you may have withdrawal symptoms or other problems if you stop taking your medicine during pregnancy. Do not stop taking Pimozide without your doctor's advice. It would be best if you did not breastfeed while using Pimozide. Consult your doctor before breastfeeding. There may be a drug interaction between Orap and other drugs such as amiodarone, amoxapine, artemether/lumefantrine, ketoconazole, azithromycin, bupropion, fluconazole, haloperidol, etc.
