Rexulti at CanadaPharmacyOnline.com

Rexulti, which contains the active ingredient brexpiprazole, is marketed in Canada by Otsuka Canada Pharmaceutical Inc.

Generic brexpiprazole is currently unavailable (at this pharmacy).

Dosage

Rexulti is available in oral tablets in strengths of 0.25 mg, 0.5 mg, 1 mg, 2 mg, 3 mg, and 4 mg.

Your doctor will determine the appropriate dose based on your individual needs and medical conditions.

Usage

Rexulti (brexpiprazole) is a prescription medication used to treat schizophrenia in adults and adolescents 13 years of age and older. It is also used as an add-on treatment to antidepressants for adults with major depressive disorder (MDD) who have not responded adequately to prior antidepressant therapies. Additionally, Health Canada has approved Rexulti for the symptomatic management of agitation associated with dementia due to Alzheimer's disease in patients with aggressive behaviour unresponsive to non-pharmacological approaches.

Mechanism of Action
Rexulti (brexpiprazole) is an atypical antipsychotic often referred to as a serotonin-dopamine activity modulator (SDAM). While its precise mechanism of action is unknown, its efficacy is thought to be mediated through a combination of partial agonism at serotonin 5-HT1A and dopamine D2 receptors, as well as antagonism at serotonin 5-HT2A receptors, and noradrenaline alpha1B/2C receptors. These interactions modulate neurotransmitter activity in the brain, which is believed to contribute to its therapeutic effects in schizophrenia, MDD, and agitation associated with Alzheimer's dementia.

Take Rexulti exactly as your doctor prescribes, following the instructions in the product monograph in Canada. Consult your doctor for personalized guidance and safe use.

The information below summarizes some, but not all, potential side effects, risks, and interactions. Before taking Rexulti, it is essential to discuss all possible side effects, risks, and interactions with your doctor or pharmacist.

• Cerebrovascular problems (including stroke and transient ischemic attack): Especially in elderly patients with dementia-related psychosis, which can be fatal.
• Difficulty swallowing (dysphagia): Can lead to aspiration pneumonia.
• Falls: Due to drowsiness, dizziness, orthostatic hypotension, or impaired motor skills.
• High blood sugar (hyperglycemia) and diabetes: Symptoms include increased thirst, increased urination, increased hunger, and fatigue. Can lead to diabetic ketoacidosis or coma.
• Increased fat levels (cholesterol and triglycerides) in your blood.
• Low white blood cell count (leukopenia, neutropenia, agranulocytosis): Can increase the risk of infection. Symptoms include fever, sore throat, and other signs of disease.
• Neuroleptic Malignant Syndrome (NMS): A rare but serious condition. Symptoms include high fever, muscle rigidity, confusion, sweating, and changes in heart rate or blood pressure.

Contraindications

The following is not an exhaustive list of contraindications. Before starting Rexulti, it is crucial to discuss all your medical conditions, past and present, and all medications you are taking with your doctor or pharmacist.

Rexulti should not be used if you:
• Are allergic (hypersensitive) to brexpiprazole or any of its ingredients.

Tell your doctor about all of your medical conditions, past and present, before starting Rexulti. This is crucial because certain conditions affect whether Rexulti is right for you. These conditions may include, but are not limited to:
• Blood disorders: Especially a history of low white blood cell count.
• Cardiovascular disease: History of heart attack, stroke, heart failure, or other heart problems.
• Cerebrovascular disease.
• Dementia: Particularly in elderly patients with dementia-related psychosis, due to increased risk of death and stroke.
• Diabetes or a family history of diabetes: As Rexulti can affect blood sugar levels.
• Dyslipidemia (high cholesterol or triglycerides).
• History of seizures or conditions that lower the seizure threshold.
• History of sleep apnea
• History of suicidal thoughts or attempts.
• Kidney problems.

• Problems swallowing (dysphagia).

Dizziness and Drowsiness
Rexulti may cause dizziness or fatigue. Avoid driving, operating machinery, or engaging in activities that require mental alertness until you understand how it affects you. Be cautious when standing up quickly, especially if you are an older patient, to reduce the risk of dizzy or fainting spells.

Drug Interactions

Before taking Rexulti, it is essential to discuss all potential risks and interactions with your healthcare provider, such as your doctor or pharmacist. For complete and up-to-date information, please visit the official Rexulti website at https://www.rexulti.com/.

Provide your doctor with a complete list of all your medications, including prescriptions, over-the-counter (OTC) drugs, vitamins, and herbal supplements. It is also essential to inform your doctor about any herbal teas, recreational drugs, or significant dietary changes you are making, as these could potentially interact with Rexulti. Rexulti may interact with other medicines, but is not limited to:
• Antihypertensive medications: Concurrent use may increase the risk of hypotension.
• Combined strong CYP2D6 and CYP3A4 inhibitors: May require a further dosage reduction of Rexulti.
• Drugs that prolong the QT interval: Caution advised as Rexulti may also have a minor effect.
• Medications that lower the seizure threshold: Concurrent use may increase the risk of seizures.
• Medications that may increase the risk of blood clots (e.g., estrogens, some antipsychotics, some chemotherapy drugs).
• Strong CYP2D6 inhibitors (e.g., quinidine, fluoxetine, paroxetine): A dosage reduction of Rexulti may be required.
• Strong CYP3A4 inducers (e.g., rifampin, carbamazepine, phenytoin, St. John's Wort): May require a dosage increase of Rexulti.
• Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin, ritonavir, grapefruit juice): May require a dosage reduction of Rexulti.

Alcohol
There are no known direct interactions between Rexulti and alcohol. However, alcohol can worsen some of Rexulti's side effects, such as drowsiness, dizziness, and impaired judgment. It is generally advisable to limit or avoid alcohol consumption while taking Rexulti.

Pregnancy and Breastfeeding

The safety of Rexulti during pregnancy has not been fully established. It should be used during pregnancy only if the potential benefits outweigh the potential risks to the fetus. Neonates exposed to antipsychotic drugs during the third trimester may experience extrapyramidal and/or withdrawal symptoms. The safety of brexpiprazole in breastfeeding is unknown. Therefore, caution should be exercised when Rexulti is administered to a breastfeeding woman, and a decision should be made whether to discontinue breastfeeding or discontinue the drug, taking into account the importance of the drug to the mother.

Use in Children

The safety and effectiveness of Rexulti have not been established in children under 13 years of age for schizophrenia and have not been established in children and adolescents under 18 years of age for major depressive disorder or agitation associated with dementia due to Alzheimer's disease. Antidepressants, including Rexulti as an adjunctive treatment for MDD, may increase the risk of suicidal thoughts and behaviours in some people aged 24 years and younger, especially at the beginning of treatment or when the dose is changed.

Use in Older Adults

Elderly patients on Rexulti must be closely monitored by their healthcare provider. Older adults, particularly those with dementia-related psychosis, are at an increased risk of:
• Cerebrovascular adverse events, including stroke and transient ischemic attack.
• Increased risk of death when treated with atypical antipsychotic drugs for dementia-related psychosis (Rexulti is not approved for the treatment of patients with dementia-related psychosis without agitation associated with Alzheimer's disease).
• Orthostatic hypotension and falls: Due to dizziness and drowsiness.
• Metabolic changes: Including weight gain and high blood sugar.
• Hyponatremia (low sodium levels): May be more susceptible.
