Prandimet (Repaglinide/Metformin) is a combined prescription medicine used to control the level of blood glucose in a patient diagnosed to have diabetic Mellitus. The active ingredients in Prandimet are Repaglinide and Metformin. Repaglinide belongs to the class of drugs called meglitinides, or simply glinides, and it works by causing the pancreas to produce insulin. Metformin belongs to the group of medications called biguanides and works by decreasing glucose (sugar) production in the liver and decreasing glucose absorption by the intestines. Controlling high blood sugar helps to prevent renal damage, sudden blindness, nerve problems, loss of limbs, and sexual function problems.

Side Effects and Precautions

The common side effects of Prandimet are extremely low blood sugar, dizziness, diarrhea, nausea, vomiting, and stomach ache.

Stop using Prandimet and contact your doctor right away if you have any of the serious side effects such as:
• Symptoms of extremely low blood sugar (severe hypoglycemia) include sweating, extreme weakness, blurred vision, sweating, tremors, stomach pain, confusion, seizure;
• Symptoms suggestive of lactic acidosis such as unusual muscle pain, trouble breathing, stomach pain, irregular heart rate, dizziness, feeling cold, or feeling very weak or tired;
• Unintended weight gain;
• Worsening of blood sugar (no significant drop in blood sugar)

Warnings and Interactions

Prandimet comes as a tablet and should be taken with a cup of water. It would be best if you took Prandimet exactly as prescribed by your doctor. It would be best to follow all directions on your prescription label and read all medication guides carefully. Your doctor may occasionally change your dose. Use the medicine exactly as directed. Prandimet is usually taken 2 or 3 times daily, within 15-30 minutes before eating a meal. Again, you should follow your doctor's instructions. You should eat your meal regularly, and if you skip a meal, do not take medicine. Wait until your next meal to prevent sudden hypoglycemia that may make you feel hungry, dizzy, irritable, confused, anxious, or shaky. If you experience hypoglycemic symptoms, you should quickly eat or drink a fast-acting source of sugar (fruit juice, hard candy, crackers, raisins, or non-diet soda). It would be best to limit your alcoholic beverages while taking Prandimet tablets to reduce the risk of lactic acidosis that may occur. Dieting, exercising, weight control, and regular blood sugar testing would be best. Follow your doctor's instructions very closely and inform your doctor if there is no improvement in your condition.

Before you start using Prandimet, inform your doctor about your medical history, especially if you have:
• Diabetic ketoacidosis;
• Heart problems such as heart failure or heart attack;
• Liver disease (including hepatitis B infection);
• Patients on NPH insulin;
• Severe renal impairment

Tell your doctor about all your medications, including prescriptions, over-the-counter medications, herbal products, and vitamin supplements. Tell your doctor if you are pregnant, plan to become pregnant, or breastfeed while taking Prandimet medication. This medication should be during pregnancy when needed and prescribed by your doctor. Insulin treatment may be preferred during pregnancy. Discuss the risks and benefits with your doctor. This medication passes into breast milk and may have undesirable effects on a nursing infant. It would be best if you did not breastfeed your child while using Prandimet for your diabetes mellitus. Reach out to your doctor before breastfeeding. There may be a drug interaction between Prandimet and other drugs such as nafcillin, tacrolimus, acetazolamide, erythromycin, ketoconazole, and gemfibrozil or NPH insulin.
